The NYC Department of Education requires all families to provide their child’s school with a CH-205 medical form before starting school.
- The CH-205 must be completed by an American pediatrician after a physical exam and list all vaccinations.
- Submit the CH-205 to medicalforms@theEcole.org by Friday, August 15
- The École can accept alternative medical documents (e.g., carnet de santé, vaccination lists, etc.) from families who have moved to the US from overseas for the first 30 days of school, provided they show the children are completely immunized. Please send all alternative medical documentation to medicalforms@theEcole.org by Friday, August 15.
Required Immunizations
New York City requires mandatory immunizations for all students. The number of required doses may differ from country to country. If you are moving from abroad, we recommend ensuring your child receives the necessary doses before arriving in the US:
Pre-Nursery, Nursery, and Pre-K
- 4 doses of DTaP
- 3 doses of polio
- 1 dose of MMR
- 3 doses of Hep B
- 1 dose of varicella
- 1-4 doses of Hib
- 1-4 doses of PCV
- Annual flu shot (administered between July 1 and December 31) → Proof must be emailed to medicalforms@theEcole.org if not listed on the CH-205.
Kindergarten to 5th Grade
- 5 doses of DTaP
- 4 doses of polio
- 2 doses of MMR
- 3 doses of Hep B
- 2 doses of varicella
6th Grade
- 3 doses of DTaP
- 1 Tdap booster (given between ages 10–11)
- 4 doses of polio
- 2 doses of MMR
- 3 doses of Hep B
- 2 doses of varicella
7th & 8th Grade
- 3 doses of DTaP
- 1 Tdap booster
- 1 MenACWY booster
- 4 doses of polio
- 2 doses of MMR
- 3 doses of Hep B

Medication Administration Forms
If your child has allergies, asthma, or a medical condition requiring medication (e.g., Epi-Pen, inhaler, antihistamines, prescription, or over-the-counter medication), their pediatrician must complete a Maternelle Emergency Care Plan or the appropriate NYC Medication Administration Form (1st to 8th Grade – available at your pediatrician’s office).
Pre-Nursery through Kindergarten
- Ask your pediatrician to complete the Maternelle Emergency Care Plan, sign it, and send it to medicalforms@thecole.org. Provide two sets of medication to your child’s teachers at the start of the school year.
1st through 8th Grade
- Ask your child’s pediatrician to complete the relevant NYC MAF(s) based on your child’s condition:
- Sign the back of the MAF and email it to medicalforms@theEcole.org
- Provide two sets of medication
The École has a Department of Education nurse based at our Elementary & Middle School Building, authorized to administer medication if the school has an MAF on file.
