- All parents must sign and submit Back-to-School forms on Focus
- Parents of newly enrolled students, and students entering Kindergarten and 6th Grade must send their child’s CH-205 medical form listing all doses of required immunization to the Medical Forms Team
- Parents whose children require medication to be administered at school must email the relevant Medication Administration Form (MAF) to the Medical Forms Team.
MAFs for 2023-2024: Allergies/Anaphylaxis Medication Administration Form, Asthma Medication Administration Form, Seizure Medication Form, Diabetes Medication Form, General Medication Administration Form.
